Daily Bolus of LR: Quick Facts on Propofol

The first bolus in our series of “Take Homes” from ACEP!  Thanks to Susie Peterson, MD for her contribution.
  • Can be used for procedural sedation, induction, and post intubation sedation.
  • Procedural sedation dose:
  • 0.5-1 mg/kg bolus
  • Onset <1 minute
  • Can re-dose in 3-5 minutes, often at half the dose, as half life is short
  • Consider using half dose in the elderly
  • Induction dose for intubation:
  • 1-2.5mg/kg IV
  • Consider using lower doses in the elderly and very sick
  • Post intubation maintenance:
  • 0.1-0.2mg/kg/min
  • Even small doses can cause apnea so BE PREPARED!
  • Keep in mind it BURNS!!! Can mix 1cc of lidocaine with 10cc’s to limit this effect.  Some papers suggest pre-treatment with fentanyl.
  • Also, this product contains components of soy and Egg so do NOT give to patients with anaphylaxis to these foods.
