Neutropenia

Neutropenia is defined as:

· A neutrophil count of less than 500 cells/mL

· A neutrophil count of less than 1000 cells/mL with the nadir expected to be below 500 cells/mL


Severe neutropenia is defined as:

· A neutrophil count of less than 100 cells/mL

Febrile neutropenia is defined as:

· A single measurement of 38.3C or greater (101F)

· Sustained measurement over 38.0C or greater (100.4F) for over 2 hours

· Rectal temperatures should NOT be taken due to the risk of mucosal trauma during the procedure

Of patients with fever and neutropenia, only about 20% will have a focus of infection found on presentation. The most common sites of infection in febrile neutropenia are:

· Lung (25%)

· Mouth and pharynx (25%)

· GI (15%)

· Skin/soft tissue/catheters (15%)

· Perineum/anorectal area (10%)

· Urinary tract (5%)

· Nose/sinus (5%)
