Daily Bolus of LR: Giardia

·         Is the most common cause of water borne diarrheal illness in the US and common cause of traveler’s diarrhea

·         Sewage contamination of the water supply is the most common etiology

·         The most common ingestion is from drinking water from contaminated streams of lakes but can also be from close contact with infected individuals (institutions, day cares, anal sex with an infected partner)

·         The Giardia Lamblia trophozoite, once ingested, infects the duodenum, jejunum or upper ileum

·         Incubation lasts 1-3 weeks

·         Symptoms typically develop the symptoms of abdominal distention with flatulence, crampy pain and frequent malodorous pale stools (non-bloody)

·         Infection can go away on its own, but if diagnosed, most patients get treated

·         Stool ova and parasites and stool antigen for Giardia can be used to make the diagnosis

·         Treatment is metronidazole 250-500mg tid for 7-10 days
